May Half Term Holiday Club

Please use the form below to request your child's Holiday Club sessions. Your contact details are requested at the bottom of the form, please ensure you provide two contact numbers in case of an emergency. When the form is completed please use 'submit' to email your request.  Once your request is received you will be sent an email confirming your booking.  Please note the HPS and LASC number is 07429807938. This is available during After School and Holiday times only.
